  • Combining insight and passion to make a difference
    Our employees work hard every day to build trust, credibility and lasting partnerships with members, their families and caregivers, advocates, community stakeholders and providers. We do this to best support members with I/DD and their families and coordinate the delivery of services that add value to their lives.

  • Assisting Families and Caregivers
    We help families and caregivers provide better care for members. For example, we are piloting a program to offer education, training and support to families struggling with how best to respond to new or escalating behaviors by their loved ones with I/DD.

  • Fostering community participation
    We address members’ evolving needs, preferences and priorities by coordinating the full range of health and support services - including housing, employment and transportation. Our goal is to help members work, go to school and participate in their community.

  • Providing extra help when needed
    Unpredictable situations, such as a family caregiver’s illness, can impact a member’s daily routine. Our enhanced respite and personal service programs (available in some states) take special circumstances into account, offering members extended services even after they’ve used their initial allocations. With this extra support, we help members stay in their homes even when the unexpected happens.

  • Promoting Choice in Services:
    Our employees uphold the values of self-direction, self-determination, dignity, and respect in every interaction with members. We take time to listen, learn and understand members’ daily lives and long-term goals. We assist them and their families in achieving their goals, supporting their independence, fostering community inclusion and promoting choice in services.

Anthem serves individuals with I/DD through innovative programs. Our affiliate plan decreased 30-day readmission rates by 52% through a transitional care program that focused on discharge planning efforts and intensive, short-term supports.
